What is Community Care?
Our service provision is monitored against the Aged Care Quality Standards to ensure our clients receive the highest standard of services.
Our highly experienced and qualified team, in partnership with you and/or your representative, will develop an optimum plan of care reflecting your values, goals and individual requirements. Consultation services with our skilled Nursing Advisory staff are conducted face to face and in your home.
Building and maintaining a positive personalised relationship with our clients is fundamental to the values of Kindred Living.
What services can I recieve?
Accessing a Home Care Package and Eligibility
- Contact Kindred Living on 08 7624 9111 and we can initiate the referral on your behalf to My Aged Care. Alternatively call My Aged Care on 1800 200 422.
- A My Aged Care representative will then contact you via phone to collect information relating to your needs.
- The My Aged Care representative will send your referral to an assessment team. The assessment team will contact you to make an appointment for a face to face assessment.
- If the outcome of your assessment deems you eligible for a Home Care Package you can contact Kindred Living.
If you are not eligible for a Home Care Package, Kindred Living offers “Fee for Service” arrangements. Further information is available by contacting our office.
